Split `PACKED_STRUCT` and `PACKED_STRUCT_UNALIGNED` macros into the

Saves comitters' daily life by avoid #include-ing everything from
internal.h to make each file do so instead. This would significantly
speed up incremental builds.
We take the following inclusion order in this changeset:
1. "ruby/config.h", where _GNU_SOURCE is defined (must be the very
first thing among everything).
2. RUBY_EXTCONF_H if any.
3. Standard C headers, sorted alphabetically.
4. Other system headers, maybe guarded by #ifdef
5. Everything else, sorted alphabetically.
Exceptions are those win32-related headers, which tend not be self-
containing (headers have inclusion order dependencies).

Onigumo 6 (r57045) introduced new onigumo.h header file, which is
required from quite much everywhere. This commit adds necessary
dependencies.
Note: ruby/oniguruma.h now includes onigumo.h,
ruby/io.h includes oniguruma.h,
ruby/encoding.h also includes oniguruma.h,
and internal.h includes encoding.h.

* numeric.c (Integer#to_f, Fixnum#to_f): move to_f method from Fixnum
to Integer.
* ext/-test-/integer/my_integer.rb: define helper class for testing
to_f method for a subclass of Integer.
* ext/-test-/integer/extconf.rb: ditto.
* ext/-test-/integer/init.c: ditto.
* test/-ext-/integer/test_my_integer.rb: examine to_f method for a
subclass of Integer.
