merges r20454 from trunk into ruby_1_9_1.
* lib/rexml/xpath.rb (REXML::XPath.first): apply documentation
patch from Ken Bloom in [ruby-core:20213].
* lib/rexml/xpath.rb (REXML::XPath.each): ditto.
