use do/while(0) around GetDBM macros
* README.EXT: wrap GetDBM with do/while(0)
* README.EXT.ja: ditto
* ext/dbm/dbm.c: ditto, likewise for GetDBM2
* ext/gdbm/gdbm.c: ditto
* ext/sdbm/init.c: ditto
